Meet the Physician

Dr. Timothy Roach, MD, DABOM

Dr. Timothy Roach is an internal medicine physician and board-certified obesity medicine specialist focused on advancing a more precise, physiology-driven approach to metabolic health.

Throughout his clinical career, he recognized a consistent gap in traditional care: patients struggling with weight, fatigue, or reduced performance were often treated based on symptoms rather than underlying mechanisms. This frequently led to variable results, frustration, and a reliance on trial-and-error approaches.

Earlier in his career, Dr. Roach served in a clinical leadership role in pulmonary transplant medicine at the University of Minnesota, working with complex, high-acuity patients. This experience reinforced the importance of understanding physiology at a fundamental level — and the limitations of relying solely on symptom-based care. It also helped shape his transition toward a more measurement-driven, systems-based approach to medicine.

In response, Dr. Roach developed a clinical model centered on objective physiologic assessment. His approach integrates advanced diagnostics — including cardiopulmonary exercise testing, DEXA body composition, FibroScan, and indirect calorimetry — to identify the specific drivers of metabolic dysfunction in each patient.

North Star Metabolic & Performance Clinic is the result of that work: a practice designed to offer patients and referring providers the diagnostic depth that traditional care often lacks.

Our Philosophy

Why Measurement Matters

Metabolic medicine has long relied on indirect measures — weight, BMI, basic labs — to guide treatment. While these markers have value, they often leave fundamental questions unanswered: Why is weight difficult to lose? Why is fatigue persistent despite normal labs? Why does exercise feel harder than it should?

We believe the answer lies in measuring what is actually happening — not just approximating it. By assessing resting metabolic rate, cardiopulmonary exercise capacity, body composition at the tissue level, liver stiffness, and a broader range of laboratory markers, we build a more complete and individualized picture of each patient's physiology.

This approach does not replace clinical judgment. It sharpens it. It allows us to move past generic recommendations and toward interventions that address what is actually limiting a patient's progress.
